Daycare Consent:
As you are aware we are a fully licensed daycare centre, to ensure compliance we must follow the Animal Welfare Act of 2018) and we obtain the required following consents and acknowledgements.
1 Off Lead/to be walked & to Mix with all other dogs.
I give my consent for my dog to be walk off lead & to be walked with all other dogs, (we use our knowledge and experience of your dogs to decide when they are on/off lead within their walks).
2 ‘Crate”
I give my consent for my dog to be crated, if necessary to allow for chill out time, /or Cleaning etc.
3 “Feed /Treats “
I give my consent for my dog to be feed onsite, with food supplied by myself (please note your dog will be fed in an isolated situation) not with others. I acknowledge that my dog may be given treats during the day, and these will be supplied whilst other dogs are nearby.
4 “Fleas”
I give acknowledge if ‘fleas’ are found on my dog, they will isolate my dog, by crating him/her of which then I will need to arrange collection.
5 “Ticks”
I give my consent) for my dog, if a TICK is found on my dog, for the team to remove the tick, (we will inform you.)
6 “First Aid”
If my dog is injured during our care we apply treatment, Dogtown will then discuss next steps, it may mean that we take the dog to your vets or emergency vet this will be confirmed.
7 “Play/Enrichment”
I give consent for my dog to interact and play, this will be with people and with other dogs. Toys and food could be included in this enrichment. A walk of up to 1 hour (if deemed suitable) at varying locations to suit the dog and weather will be included as enrichment.
8 Transport
I give my consent for my dog to travel and be transported, within a Dogtown approved vehicle. I acknowledge that my dog will travel within a crate, and they may be sharing this space with other dogs that are matched for their temperament.
