Veterinarian Consent Form:

To Whom It May Concern:

As you are aware we are a fully licensed daycare centre, to ensure compliance we must follow the Animal Welfare Act of 2018. we must obtain the required following consents and acknowledgements, or we will not be able to provide daycare services for your dog.

Vaccination Consent: (to contact my Vet’s Directly/on behalf of myself)

I give my consent for Dogtown Ltd to be sent any relevant information held on my dog’s Medical History e.g., vaccination records as part of their Daycare attendance.

Treatment Consent:

I acknowledge that if my dog becomes ill whilst in your care, non-emergency we will contact you/owner and agree next steps. If we are unable to contact you, we will contact your chosen vet. For urgent care we contact our registered emergency vets and treatment may be provided by our agreed vets

Euthanisia:

If in the highly unlikely your dog is injured and found to be suffering to such an extent, and in extreme pain that death is expected; at the discretion/advise of the veterinarian, they may be humanely euthanised.
